Nutrition Case Management Program
The Nutrition Case Management program partners with individuals with a Registered Dietitian (RD) who will do home visits, focused on nutrition and physical activity. Together, the client and RD will create a care plan to assist clients in reaching their health goals.
The RD will:
- Assess client’s nutritional concerns and needs.
- Work with clients and their families to set personal health goals related to nutrition through home visitation appointments.
- Coordinate care with doctors, mental health providers, specialists, case managers, social services providers, and others.
- Refer to resources, including but not limited to, food, dental, physical activity, housing, emotional and social support that may be needed.
Our culturally diverse Nutrition staff is welcoming to all communities. Individuals of childbearing age, children, and adolescents who are Medi-Cal members and have a nutritional concern are eligible for Nutrition Case Management. This is a voluntary program and free of charge.
