NHF Program
The Nursing Healthy Families (NHF) Program is committed to serving women, children, teens and their families in Monterey County by improving access to comprehensive, quality health care, and focusing on prevention and early intervention strategies.
Our NHF Program will partner individuals with a registered nurse case manager who will visit them in their home. The registered nurse case manager will help provide education, community resources and support regarding the following:
- Pregnancy/post-partum care
- Infant/Child health
- Immunization education
- Family Planning
- Exposure to communicable disease education
- Physical and/or emotional health concerns for individuals of any age
- Chronic disease education
- Nutrition
- Accessing health care
- Feelings of sadness/depression
- Loss of a loved one
Our culturally diverse NHF staff is welcoming to all communities. Individuals of child bearing age, children and adolescents who are eligible for Medi-Cal can receive NHF Program services. This is a voluntary case management program and free of charge.
