Provider and laboratory reporting of diseases and conditions mandated by Title 17 of the California Code of Regulations is important to help protect the health of communities.  Laboratories should report electronically through the California Department of Public Health's (CDPH) Electronic Laboratory Reporting (ELR) system.  Providers should ideally report electronically or through CDPH's Provider Portal.  In lieu of utilizing these systems, providers may fax Confidential Morbidity Reports to the fax number listed on the forms below.
