The Spring 2026 Mental Health consumer perception survey period will take place from coming soon
All patients receiving mental health out-patient services through Short Doyle / Medi-Cal providers during the five-day period should be surveyed.
Each clinic/provider has the option to administer and collect survey data via paper survey forms or via an online survey link. Six unique Online Survey links, QR codes and flyer templates will be disseminated prior to May 16, and links will only be active during the data collection period.
As we look toward Consumer Perception Survey week, we wanted to send along a few important reminders -
- The UCLA online survey portal will collect data from coming soon
- Only data submitted during this date range will be accepted.
- Survey data can be collected both via paper surveys and the online portal.
- PDF’s Surveys can be found on our website.
